Types of Breast Augmentation
While the majority of plastic surgeons perform breast enlargement using breast implants, Geneva, Switzerland plastic surgeon Dr. Schlaudraff offers women another choice - they can choose natural breast augmentation using cell-enriched fat grafting. He will consider your individual body type and personal preference when determining which method is best for your breast augmentation.
Natural Breast Augmentation
Women who have excess fat in the abdomen, flanks, or hips can now benefit from the latest technologic advances in fat harvesting, adult stem-cell enrichment, and fat grafting to fulfill their wish for a breast augmentation using their own tissue instead of implants. This revolutionary method of natural breast augmentation, also called "fat transfer breast augmentation," combines lower body sculpting through liposuction and relocating the fat to the breasts, providing breast enlargement that looks and feels completely natural. Traditional Breast Augmentation with Implants
For women without significant excess fat or for those who wish to obtain a very pronounced increase of their breast volume (more than 2 cup sizes) silicone filled breast implants remain the treatment of choice. The implants are usually introduced through a small incision around the areola or in the skin fold under the breast and placed under the breast gland (subglandular) or under the pectoralis muscle (subpectoral). The additional volume of the implant enhances breast fullness, shape, and consistency.
Breast Enlargement Candidates
Breast augmentation appeals to a wide range of women, including younger women who are unhappy with breasts that are not fully developed or who wish to enhance their female curves. The procedure also can be performed for more mature women whose breasts have lost volume and begun to sag or droop. In these cases, breast enlargement is often combined with a breast lift.
Your Consultation
During the consultation, Dr. Schlaudraff (board certified by the Swiss and the European Board of Plastic Reconstructive and Aesthetic Surgery and a member of ISAPS) will find out how you feel about your breasts and how you want them to look after surgery. He will then examine your breasts and carefully evaluate the quality of the skin, fatty tissue, breast glands and pectoralis muscle. Measurements for the volume, width, shape and projection of the breasts are taken and special note is made of the nipple position, the height of the mammary fold and of breast asymmetries. These details will influence the choice of breast implants, the optimal implant surface (smooth vs. textured), implant position (subglandular vs. submuscular) and implant shape (round vs. anatomical).
Whether they desire breast augmentation with fat grafting or breast implants, Geneva, Switzerland women who come to the Concept Clinic will be advised by Dr. Schlaudraff about their full range of options.
Surgical Procedure
Breast augmentation is performed in a state-of-the-art surgery theater under general anesthesia. When using implants, Dr. Schlaudraff makes a small incision around the areola or in the skin fold under the breast and creates the pocket for the implant either under the mammary gland (subglandular placement) or under the breast muscle (submuscular placement).

These surgical decisions will depend on what Dr. Schlaudraff feels is most suitable for the patient. After insertion of the breast implant the incision is closed with resorbable skin sutures and a protective dressing is applied. The actual procedure takes 1 to 2 hours. The surgery might be organised as an outpatient procedure but often the patient is advised to stay in the hospital for 1 day for optimal recovery.
Natural breast augmentation combines lower body sculpting through liposuction and transferring the fat to the breasts. In a first step, Dr Schlaudraff performs ultrasound-assisted liposuction through tiny incisions in order to harvest a sufficient amount of fatty tissue. After the process of cell enriching, the fat is then grafted to the breast through barely visible injection points. The actual procedure takes 2 to 4 hours. The surgery might be organised as an outpatient procedure but often the patient is advised to stay in the hospital for 1 day for optimal recovery.
Recovery and Results
The soreness during the first days after surgery will vary for each patient and is usually described as a tension and discomfort that limits upper body movements. After 1 to 2 days your bandages will be removed and the breasts are then stabilized by a sports bra that will need to be worn for about 8 weeks. Taking a shower and traveling is allowed within a few days postoperatively.
You may return to your normal light activities 2 to 3 days after surgery and you will be able to make full physical efforts after 7 to 10 days after surgery, except for strenuous arm movements. If your job requires lifting, you may return to work in the same timeframe but you will probably have lifting restrictions. Scar treatment and massaging of the breast is started after 2 to 3 weeks.
The final result will be seen after 8 to 12 weeks when swelling has resolved. At this time you may also resume strenuous physical activities (such as diving, climbing, sauna), but you must avoid sun exposure of the scar for 6 months.
A Note on Breast Exams
The risk of developing breast cancer is not increased by breast implants or fat injections to the breast. Whether an implant is placed under or over the muscle, it does not interfere with your ability to detect lumps in a self breast examination. Radiological exams for detection of breast cancer are possible, as well, but you should inform your physician about your breast augmentation surgery before the exam. They should be performed by your gynecologist or radiologist as part of a routine cancer check-up or might be asked for before surgery to rule out suspicious lesions.
